ABSTRACT
The effect of hormonal therapy on survival continues to be debated. However, doubts about the value of immediate versus deferred treatment are not confined to survival issues alone. With earlier diagnosis, many could be on treatment for many years. There is an increasing awareness of the toxicity of hormonal therapy, which has to be balanced against any benefits. The decision when to start therapy and which treatment to use depends on the balance of benefit and risk.
