Patients request medication changes for a variety of reasons including perceived inefficacy. However, without clear treatment goals, the clinician runs the risk of prematurely ceasing a beneficial treatment or over-prescribing. Clinicians and patients need a shared understanding of how medication changes will be helpful. This chapter reviews a technique for aligning treatment goals around medication changes. Pitfalls in the use of this technique are reviewed.
