Tom was initially referred by his primary care physician and his neurosurgeon. However, additional referral sources were added before the fi rst appointment including his workers’ compensation attorney and an attorney representing him in a lawsuit against the driver of the other vehicle. Th e patient received a severe head injury with a basilar skull fracture in mm/yyyy. Th is was followed by several days of posttraumatic amnesia. Th is neurocognitive evaluation was requested to establish the patient’s current level of neurocognitive functioning and to aid in treatment planning.
