ABSTRACT

This chapter describes the investigation of possible causes, analyses the effects of the remedial measures implemented and examines connections with theories of risk-homeostasis and habit-intrusion. Heathrow Express is a state-of-the-art railway opened in 1998 by the company that operates London’s Heathrow airport. Its trains run from underground stations serving Heathrow’s terminals non-stop to Paddington Station in the centre of London. Brief free-text descriptions had been recorded of each incident. Accidents while boarding outnumbered those while alighting. A few passengers admitted not having been paying sufficient attention or not having noticed the gap. Female passengers outnumbered males in these accident records by a factor of 2:1. Generally the range of their estimated ages was between 20 and 70 years, with just one child and one or two more elderly. It appears that these accidents most commonly occur to fit and healthy adults.
