ABSTRACT

This chapter reviews the differing approaches to case management with the chronically mentally ill (CMI) and relevant research findings. It outlines clinical guidelines for a clinical case management model. The chapter describes the plan implementation and coordination functions within this model. It examines the priorities that must be set in motion, including appropriate training, before appropriate case management can become a reality for the CMI. The case management approach that appears to be most effective in creating an environment for sustaining a reasonable quality of life for a CMI client is one that is clinically based. The use of student interns as case managers reflects a prevalent misunderstanding in service delivery of the potential conflict that can arise when CMI clients are exposed to the multiple stresses of the community. The case manager must be part of a treatment team that includes housing and entitlement specialists, psychiatrists, physicians, social workers, and psychologists.
