ABSTRACT
The second stage of the conversation is the analysis of any issues seen or raised. This is where the principles of ‘Just Culture’ come into play (see Figures 4.1, 4.2 and 4.3). Issues might include failure to wear PPE, poor housekeeping, driving a forklift truck unsafely, or perhaps a tool box talk or brief that lacked impact.
