ABSTRACT

Assessments are an essential prerequisite to caring for frail and ill older people. In the UK, formal assessment of continuing care need is required for: (i) placement in Care Homes; (ii) review at the time of transfer; and (iii) review at stipulated intervals thereafter on an ongoing basis. Health and social care assessments result thus facilitate the placement of an older person in a suitable ‘fit-for-purpose’ facility. The continuing care assessments assist with the planning, implementing, and adjusting of care provision once the resident has been admitted, as they form baseline information for the Care Home. Continuing care assessments are carried out by the NHS and local authorities.
