ABSTRACT

To explore how your primary care team might work together to improve patient safety in your practice you can review a case (or cases) with which your team has been involved. You may prefer to use the two example cases at the start of this chapter. Read through them quickly before working on the problem case exercise (see page 228), or photocopy Table 1.1 on page 7 and develop your own problem case with incidents from your own practice. This process of working together leads to better risk assessment, a deeper insight into potential problems and more effective solutions. Include in your team people who will join in the problem-based learning discussion and be part of the solutions.
