ABSTRACT
Review a case (or cases) with which your team has been involved to explore how your primary care team might work together to identify its learning needs and improve patient care. You may prefer to use the two example cases at the start of this chapter or read through them quickly before working on your own practice needs assessment based on your own problem case, using the Table on page 31. Include in your team people who will join in the problem-based learning discussion and be part of the solutions.
