ABSTRACT

Homefield Place is jointly funded and staffed by Health and Social Services. A Community Service Manager for the elderly of Seaford was jointly appointed by the agencies to have overall responsibility for Homefield Place and its services. Carers, meeting weekly, have expressed concerns as to what would happen if they were taken ill or died while out of their home and nobody knew of their dependent person at home. Homefield Place has brought professionals together, resulting in both the client and the carer receiving more appropriate assessment treatment and a care programme more finely tuned to their respective needs. Homefield Place does not provide long-term continuing care. Each unit has purpose-developed facilities providing a very high quality of individual rooms, some with en-suite facilities with a homely environment. The resource centres, as developed, have firmly located local facilities for local people within the differing communities of East Sussex.
