Primary care oral anticoagulant management can be as effective as hospital-based clinic. The Birmingham model of primary care anticoagulant management, the only model with a robust evidence, has become an acceptable alternative to hospital outpatient management.1 This is a nurse-led clinic supervised by a general practitioner using near-patient INR (international normalised ratio) testing and computerised decision support software for support regarding the dose and advice on recall.
