ABSTRACT

This chapter describes the importance of population health management. It identifies the role of information technology in population health management. The chapter articulates the value of activated and engaged patients. It describes how health information technology can improve patient engagement. It addresses two key strategies in the national effort to improve value delivered by healthcare institutions in return for the commitment of funding and other resources that is made to them. The two key strategies are: Improvement in population health and Improvement in patient engagement. The concept of population health has gained a lot of attention in recent years and refers to "the health outcomes of a group of individuals, including the distribution of such outcomes within the group". Often these populations are defined by geographic region. Population health is important because it focuses on the health of everyday individuals and the groups to which they belong.
