Compounded drug for pediatric may have some problems in drug interactions, stability and suitability. This study aimed to analyze problems from compounded drug prescriptions for pediatric treatment in a secondary health care facility in Bandung. The study was conducted retrospectively to 2,114 pediatric prescriptions from August to December 2015. Problems were analyzed based on literature. There were 405 prescriptions (19.16%) for pediatric containing 455 compounded drugs, divided into 357 (78.46%) oral drugs and 98 (21.54%) topical drugs. Problems detected on oral compounded drugs were drug interaction (16.81%), impaired stability (18.49%), antibiotic mixture (46.50%), and crushing of coated tablets (1.96%). In 19 prescriptions contained substances that were not suitable for the pediatric patient. Compounded drugs prescribed for pediatric in the health care facility may have some problems primarily in change of potency in antibiotic mixture, stability of drug, and drug interaction. These should be considered in prescribing practice and compounding preparations.
