ABSTRACT

Almost 5% of hospitalized patients are being affected by Adverse Drug Events (ADEs). The aim of this study was to identify the potential level of risk, find the cause of the problems, and provide recommendations to improve patient safety. Incident reports were classified by type of events, and each type was analyzed using the Risk Assessment Scale (RAS). The high- and extreme-risk scales were analyzed using the Root Cause Analysis (RCA). There were 22 incidents of patient safety events, in which 45.5% were related to drug administration. According to the RAS, 40% of incidences were at low risk, 50% at moderate risk, 10% at high risk, and none at extreme risk. The RCA showed that high risk of ADEs was caused by the non-compliance of staff with the hospital standard operating procedures. More than half of the ADEs were in moderate to high risk, thereby rendering the role of reminding and emphasizing necessary to improve staff compliance.
