ABSTRACT

The workshop was designed by the co-chairs to focus on the investigation of human performance issues associated with a major aviation accident / incident. The main aims were to: review and provide practical experience in the use of human performance investigation methods, and evaluate methods and determine where further development may be required. Informal indications were that most participants found the workshop to be enjoyable and worthwhile. They enjoyed the case studies, and there was substantial and interesting discussion on many issues. Some of the participants would have preferred a more "developmental" approach, but the facilitators had to focus on the requirements of the majority. In future exercises of this type, it would be worthwhile to run "training" and the "development" workshops on this topic separately.
