ABSTRACT
This chapter is concerned with signals passed at danger (SPADs) which are caused by some form of driver error (termed Category A SPADs in the UK) and are one of the key railway risks focused on by the UK railways (RSSB, 2005). There has been concern about an increase in the risk of SPADs when returning to work following rest days.
