ABSTRACT

Healthcare settings, which are characterized by numerous handos across care providers and sites, multiple opinions, interactions across various specialties, and other complexities provide rich opportunities for communication breakdowns. Cultural, legal, and organizational barriers further exacerbate these problems in sharing information. Creating the conditions for open communication means both developing capacity with structuring and teamwork, as well as removing opportunities for confusions in work ow that might lead to poor monitoring, awed intervention, or incorrect dosage.
