ABSTRACT
A report published in 2000 by the Institute of Medicine highlighted the frequency of patient deaths from medical errors and
alerted the U.S. public to the need for quality assurance (QA) (Kohn, Corrigan, and Donaldson 1999). Unlike some specialties, however, QA is well established in radiation therapy, resulting, at least in part, from the eld’s quantitative nature.
