ABSTRACT

The medial (tibial) and lateral (fibular) sesamoids of the great toe are contained within the tendons of the FHB, corresponding to its medial and lateral heads.1 They articulate with corresponding facets on the plantar surface of the first metatarsal head, separated by an osteocartilaginous crista, and form part of the plantar plate complex, which also comprises the plantar portions of the abductor and adductor hallucis tendons, the intersesamoidal ligament, the joint capsule and the collateral ligaments. The plantar plate complex inserts onto the base of the proximal phalanx; the flexor hallucis longus tendon runs between and plantar to the sesamoids and inserts onto the distal phalanx (Figs 124.1 and 124.2)

Turf-toe injuries and sesamoid fractures share similar mechanisms of injury, but have many distinct clinical features, and are considered separately here.
