INTRODUCTION Before starting, the reader should be aware of certain problems in discussing the subject.

First of all, a great deal is known (relatively speaking) about the microscopic structure of the bladder, urethra and pelvic floor, but as one works back proximally through the innervation of the lower urinary tract to the spinal cord and up to the brain, and as one turns more from structure to function, so knowledge of the subject becomes exponentially less.
