As mentioned in Chapter 3 there is continuum of care that has overlapping portions during the total sum and loss stages.
THE PROCESS OF TRANSFERRING THE CLIENT FROM HOSPITAL TO HOME AND COMMUNITY By now the client is in the process of being discharged from hospital. The hospital clinicians are, from their own disciplines’ perspectives, preparing discharge summary reports. These reports will be kept as a permanent record at the hospital and will also be requested by whoever will be funding the community-based portion of the clients care as well as potentially subpoenaed by the litigators.
