Unless an MIB application form6 with supporting information and documents is lodged, the MIB can refuse to deal with your client’s claim. The forms are straightforward to complete. It is wise to lodge such an application form as early as possible to enable negotiations to commence. It must be lodged no later than 14 days after the commencement of proceedings.7
