ABSTRACT

Registries are cohorting tools that help to identify groups of patients with shared characteristics and needs. The applied population health approach uses the registry inclusion rules to govern population definitions and registry metrics to drive consistent decision support and clinical practice across an organization. The wellness and prevention registries identify patients for primary prevention efforts in the general patient population, primary care, or patients who are at risk for developing chronic disease based on clinical assessments or family history. Some medications have known risk of side effects or adverse events. Patients taking these types of medications may be placed on a routine monitoring schedule to reduce that risk. “The Who” for Laboratory Monitoring for Safety of Persistent Medications includes patients with an active ambulatory medication order for Digoxin, Angiotensin-Converting Enzyme Inhibitor, Angiotensin II Inhibitor, certain types of diuretics, statin or thyroid replacement therapy.
