ABSTRACT
History A 43-year-old man is referred to the medical team by A&E after presenting with lethargy and shortness of breath. He was found in a semi-conscious state outside the local fast food restaurant and brought in by ambulance. After a few hours of intravenous fluid resuscitation, he came round and gave doctors there a 2-month history of progressive shortness of breath and tiredness after walking half a mile.
