ABSTRACT

This chapter explores case management and dementia care. Two case studies are used to illustrate challenges faced by individuals living with dementia and their families: the cases of Annie and Beryl and their caregivers. R. Boaden et al. described case management as follows: Case management for frail elderly people aims to combine both preventive and responsive care for patients at high risk of deterioration in their health. A plethora of case management models exists in the National Health Service. Essentially case management consists of four components: assessment, care planning, review, and a case manager. In the assessment process involving people living with dementia, there is a chance that those caring for them will focus mainly on cognition and functioning. The package of care that is thought to be appropriate to the person's needs is dependent on the outcome of the assessment. The aim of care planning is to co-create a plan which the individual understands and is in agreement with.
