ABSTRACT

The Community Partners Track provides the requirements for the workflows and the tools needed for Community-Based Social Service Organizations aka Community-Based Organizations (CBOs) to achieve the goals of the Connected Communities of Care (CCC). The work in the Community Partners Track can generally begin once the CCC key decision makers have completed the prerequisites of the Clinical Providers Track. The CCC Community Partners Track lead should work with the CBO sites’ mid-level managers and frontline staff to determine the best times to conduct the pilots. The community food pantries, homeless shelters, crisis centers, and transportation service providers are the lifeline for many vulnerable and underserved community residents. Access to nutrition services will depend on the funding available under a specific program at a CBO. Advocacy groups are a helpful resource when needing assistance to secure funding and in creating key partnerships in the community and with government groups.
