ABSTRACT
To understand design error, it is necessary to understand the design process. At the outset of design of a major process plant, there will be a specification or conceptual design stage. The starting point will be a certain need, for example, the need to develop an oil field or to provide new capacity for a plastic production plant. Today the need for refinery or petrochemical plant construction is rarely pure greenfield; it is usually alongside an existing plant.
