Lung cancer is the leading cause of cancer death in the United States and is increasing in incidence worldwide. The vast majority of lung cancer cases can be attributed to cigarette use with several other environmental agents accounting for the remainder of the cases. Human lung cancer consists of four main subtypes, small-cell lung cancer, squamous-cell lung cancer, lung adenocarcinoma, and large-cell lung cancer. Specific genetic and molecular attributes distinguish each type of lung cancer and indicate that treatment of each lung cancer type will require distinct therapies. In this chapter we briefly describe the major subtypes of lung cancer, consider the environmental agents that induce this neoplasm, and describe the genetic changes that have been linked with lung cancer susceptibility. The underlying molecular alterations involved in control of the multistep process of lung tumorigenesis are considered. The continued elucidation of the molecular and genetic changes observed in human lung cancer is critical to increase our understanding of this disease and to offer new avenues for improved treatment of lung cancer patients.
