ABSTRACT

Radical cystectomy remains the gold standard for invasive bladder cancer and the open technique with urinary diversion is the most common procedure. However, the numbers in the series are limited, the follow-up periods are short, and various procedures have been described concerning radical cystectomy and urinary diversions. This chapter describes our technique of laparoscopic radical cystectomy in males with ileal neobladder, named Minimally Invasive Laparoscopic Neobladder (MILaN). At this point only the deep dorsal vein complex and the urethra fix the vesico-prostatic complex to the pelvic floor. The authors describe a technique of ileal neobladder partially performed extracorporeally, known as MILaN. It is possible to perform the neobladder reconstruction and the uretero-neobladder anastomosis completely extracorporeally. In this case, it is better to perform the skin abdominal incision in an inferior position with respect to the umbilicus, only the urethral-neobladder anastomosis is performed intracorporeally.
