ABSTRACT

Broad ligament are rare types of fibroids and may have an unusual clinical presentation as well. Currently, there are limited data on the ease of their safe laparoscopic management. Their management becomes a challenge for a surgeon because of the abundant vascularity, huge size, and location. Furthermore, there are risks of ureteric injury. This chapter is an attempt to ease the diagnosis and management of such fibroids. The chapter discusses the better novel techniques to approach the case of broad ligament fibroids through the laparoscopic route with minimal blood loss. It focuses on the “Backyard theory”, the role of vasopressin, and the newer entry technique, the Jain point, to tackle such large broad ligament fibroids. It also discusses various anticipated complications that one can encounter and the tactful ways to prevent such intraoperative complications.
