ABSTRACT

Single-centre case-based study introducing the surgical technique of combined fronto-orbito-facial advancement as a single-step procedure. In general, the craniofacial unit of the authors has extensive experience in orbital and facial mobilisation, stemming from 176 cases, 71 of them suffering from Crouzon's and Apert's syndromes. A satisfactory postoperative course is reported in all patients, with no serious complications such as cerebro-spinal fluid fistula, frontal bone flap loss, or meningitis. Postoperatively the dental occlusion remained normal in four patients, two followed-up for 3 years, one for 2 years and one for 1 year. Exorbitism was corrected in all cases, giving a more anatomical correction than previous techniques due to simultaneous advancement of the whole orbital ring. The correction of craniofacial midface malformations has undergone considerable development over the last decades to improve/correct problems. In order to overcome the problem of ascending infection, a galeo-pericranial flap combined with bone grafts to cover the skull base defect has been described.
