ABSTRACT

A 15-month-old boy was found unresponsive at his uncle's home when his grandmother came home from the store. Emergency medical services found him apneic but with a pulse. He was intubated and transported to a pediatric trauma center where his primary survey showed an intubated toddler with equal breath sounds, peripheral pulses, and a heart rate of 80. On secondary survey, it is noted that he will withdraw from pain and a Glasgow Coma Scale (GCS) score of 6 is assigned E(1) V(1) M(4) (see Case 40). He has a large occipital hematoma with a palpable defect and some oozing blood from it. There is also bruising on his chest and lower extremities. His grandmother states that the child was well before she went to the store. She was gone for about 30 minutes and left the child with his uncle. The child is healthy and takes no medications. A CT of his head is shown next.
