ABSTRACT

The eye is sometimes called the gateway to the human body. Various systemic diseases have different ocular presentations, which sometimes may even herald the onset of the systemic disease. The cornea can be involved in a variety of autoimmune, skeletal and connective tissue disorders and endocrine and metabolic diseases. The patient was treated with topical and oral broad-spectrum antibiotics and artificial tear drops and was taken for an emergency tectonic patch graft. Keratoconjunctivitis sicca is the most common ocular manifestation in rheumatoid arthritis (RA) and may present as punctate keratopathy, mucus stranding and filamentary keratitis. He also gave a history of recurrent sinusitis for which he took on/off treatment from a local physician. A gentle scraping was performed at the site of corneal infiltrates and some specimen was also collected from the area of sclera necrosis using a cotton-tipped applicator.
