ABSTRACT

Preservation of fertility provides options for creating biologic children in those undergoing gonadotoxic therapy or other sterilization procedures due to necessary medical treatment, resulting in iatrogenic infertility. Fertility preservation is most successful prior to the initiation of gonadotoxic therapy; therefore, prompt referral at the time of diagnosis is optimal. Testicular tissue cryopreservation is offered to prepubertal males only under research protocols. Gonadotropin releasing hormone agonists have not conclusively been identified as a means of fertility preservation in the adolescent patient and should not be offered in lieu of other standard of care options when available. Iatrogenic infertility has a lasting and significant impact on mental health and relationships; therefore, collaboration with mental health provider should be considered.
