ABSTRACT

The “WHY DO PEOPLE DO WHAT THEY DO” Workshop will introduce a Human Factors Analysis Toolkit that is used by an international energy company’s incident investigators to analyse behaviour, and to subsequently put in place more effective recommendations to prevent a similar accident happening again. The “Toolkit” comprises an ABC analysis for intentional behaviours and a human error analysis for unintentional behaviours. The Human Factors Analysis Toolkit was developed to address shortcomings with the existing root-cause analysis methodology, which was recognised as being very effective at identifying what happened and addressing technical failures, but not so successful in consistently identifying why people behaved as they did. An ABC analysis begins by defining the antecedents of the behaviour. Antecedents can be the presence or absence of factors such as suitable tools and equipment, other peoples’ example and procedures.
