ABSTRACT

Urethral diverticulum (UD) is a periurethral cystic structure that is connected to the urethra via an ostium. They can present as anterior vaginal wall masses that cause a wide array of lower urinary tract symptoms, dyspareunia, and pelvic pain. Diagnosis of a UD requires a complete history, an astute physical exam, and diagnostic testing that can include cystoscopy, voiding cystourethrogram, and MRI. Urethral diverticula are most often benign, but malignancy within the diverticulum can occur. Management of UD can include the observation of asymptomatic patients or surgical management of bothersome patients. Surgical management includes marsupialization, endoscopic unroofing, fulguration, and excision most commonly through a transvaginal approach.
