Evolutionary processes affecting the human genome lead to a customized physiology that confers a survival advantage. Environmental conditions that influence the human epigenome and manifest phenotype are part of this evolutionary story, but are influenced by the impact of technology. It is in this setting that lifestyle practices and interventions are interpreted in the cardiometabolic space. Understanding the mechanisms accounting for the significant rise in prevalence of cardiometabolic drivers, particularly insulin resistance, in the very recent past can shed light on optimal prevention using lifestyle medicine. Key mechanisms with an evolutionary precedent include:
