ABSTRACT

The aviation system is inherently safe; yet around-the-clock aviation operations push the limits of pilots and other safety-critical personnel. This chapter examines how fatigue manifests in aviation near misses and crashes and discusses a number of occurrences in which fatigue was causal or contributing to the event. The chapter concludes with a discussion of lessons learnt from transportation safety investigations that uncovered fatigue issues and how such investigations can inform the development of countermeasures to reduce the incidence of fatigue-related events.
