Hand surgery is specialist work. It requires knowledge of the complex static and functional anatomy of the hand. Hand surgery in Europe is a recognized specialism in seven countries. It refers to a surgeon with a proven experience and interest in all aspects of hand surgery; and to one who works within an infrastructure that can provide optimal aftercare. Remnants can sometimes be used as spare parts. If the injury is limited and the treatment fits within the competence of the Emergency Department physician, the finger or hand can be anaesthetized. Disinfect and cover, but leave a large field to operate. More than elsewhere in the body, hand surgery requires a non-traumatic technique. Lift the skin with hooks and not with forceps. In case of tissue loss, especially combined with an open joint or exposed tendons, bone or nerves, a transposition or transplantation of skin is indicated.
