ABSTRACT

Diastema and midline shifting are common aesthetic complaints of patients seeking for orthodontic treatment. There are several approaches used in the treatment of diastema and midline shifting, one of these is fixed orthodontic appliance with standard technique. A 22-year-old female patient came to Universitas Sumatera Utara dental hospital with chief complaints of midline shifting and spacing between upper central incisors. Patient had good general health, no allergies and trauma were shown. Extra oral examination showed concave profile and symmetrical facial, competent lips, and no bad habits were shown. Diastema is present with overjet and overbite 11/41, 21/41 = 2 mm. The upper median line shifted 1.5 mm to the left. Cephalometry analysis showed class I skeletal malocclusion. Panoramic radiographic analysis showed missing teeth 36 and 46. The goals of orthodontic treatment are to correct the upper central incisors diastema, coincide the dental midline with facial midline, restore missing teeth 36 and 46, and achieve good interdigitation. A fixed orthodontic treatment with an edgewise system was begun, initially levelling and aligning with multiloop archwire on upper and lower teeth, retraction was made simultaneously in closing the diastema and shifting the upper median line 1.5 mm to the right. The correction of diastema, midline shifting, and a good interdigitation have been obtained after 8 months of the orthodontic treatment. Orthodontic treatment with standard technique is effective and efficient for the management of diastema closure and midline shifting.
