ABSTRACT

Atypical fibroxanthoma (AFX) is a relatively uncommon low-grade malignant spindle cell tumor of fibrohistiocytic histogenesis. The tumor is most often found on the head and neck on sun-damaged skin in elderly patients. Clinically it presents as a solitary asymptomatic nodule with ulcerative features, which can grow rapidly. Although extremely nonspecific, dermatoscopic observation of AFX mainly found reddish and whitish areas and an atypical polymorphous vascular pattern characterized by the concurrence of various structures: linear, dotted, hairpin, arborescent and highly tortuous vessels irregularly distributed over the surface of the lesion.
