In vivo reflectance confocal microscopy (RCM) is a promising tool for the differential diagnosis of pigmented lesions in the genital and oral mucosa1-3 and in particular for the differentiation of early melanoma from the more frequent melanosis.1,2 Mucosal melanoma is characterized by four major features: presence of pagetoid bright cells in the epithelium (mainly roundish or spindle shaped with plump body), high density of basal hyper-reflective dendritic cells, loss of the normal architecture of the chorion papillae, and sheet-like proliferation of atypical cells in the chorion (Table 27.1).1,2 However, melanoma features differ according to the growth phase.
