Incisions were often positioned directly over the tumour irrespective of the position, which led to cosmetically unpleasant scars on many occasions. In some cases, more complex incisions are used often in combination with specific oncoplastic techniques with displacement of surrounding tissues. The crescentic incision is perfectly placed to resect tumours superior to the areola. The inclusion of a paddle of skin enables a small uplift which can be used effectively in the ptotic breast. The ‘batswing' incision is an underused incision for larger tumours above the areola and can provide an excellent cosmetic outcome for the older patient with significant ptosis. The Wise pattern incision can be used to access virtually any area of the breast with wide margins and provides a fantastic exposure and often an improved aesthetic of the breast.
