ABSTRACT

Lipomodelling plays multiple roles in the breast surgeon's toolbox, and it can be used as a primary means of reconstruction, or as an adjunct to other procedures. This chapter discusses the technique of fat grafting, the indications and considerations for its use and the complications associated with this technique. The injection technique used is the most critical aspect of lipomodelling to ensure and maximise fat survival. Lipomodelling frequently requires multiple surgeries to obtain the desired result. Therefore, ongoing discussions and assessments are undertaken over a period of time, and each time consent for a procedure is undertaken, a full and detailed explanation of all potential risks and benefits should be discussed. It is important to delay the fat-grafting procedure for at least 4 to 6 months after the first stage of surgery/reconstruction in order to avoid the presence of any cavity or tissue undermining, as injecting fat into the dead spaces leads to fat necrosis and an unpredictable final outcome.
