ABSTRACT

Packed red cells and platelet concentrates are frequently required for the transfusion support of patients undergoing stem cell therapy. These components have to be selected to ensure that special transfusion patient requirements are met. This includes leucocyte reduction, irradiation, CMV compatibility, hepatitis E negative status, and sometimes washed or HLA-matched components. The concept of patient blood management aims to reduce the patient need of transfusion by proactive management of pre-existing anemia and bleeding risks. Red cells and platelet transfusions are usually guided by patient blood count. There are now trial data and guidelines to direct the use of these components in the stem cell transplantation setting. Modern transfusion practice is safe. However, transfusion complications continue to occur, many of which are avoidable. Eliminating unnecessary transfusions is an important step toward safer transfusion practice.
