ABSTRACT

This chapter describes the investigations, premedication, intravenous access and monitors of the carotid angioplasty. It also describes the positioning, intravenous fluids, reversal or elective mechanical ventilation and postoperative analgesia of the carotid angioplasty. The chapter focuses on the intravenous fluids namely, crystalloids and blood and blood products. It discusses the drugs like aspirin/clopidogrel/dipyridamole, heparin, lidocaine, vasopressors, intraventricular (IV) paracetamol and antiemetics. The chapter explains the intraoperative concerns like bradycardia, hypotension, or hypertension, risk of massive blood loss and stroke or myocardial infarction. It also focuses on the postoperative concerns like stroke or myocardial infarction, hypotension or hypertension, cerebral hyperperfusion syndrome, cranial nerve injury and dysfunction, pain and postoperative nausea and vomiting (PONV) and abnormal coagulation profile following major blood loss. The chapter also discusses the patients with massive intraoperative blood loss and hemodynamic instability may require mechanical ventilation. It also explains that the patients can be extubated with controlled intraoperative management.
