ABSTRACT

This chapter describes the investigations, premedication, intravenous access and monitors of the epilepsy surgery. It also describes the induction, intravenous fluids, reversal or elective mechanical ventilation and postoperative analgesia of the epilepsy surgery. The chapter discusses the goals of anesthesia to facilitate rapid and smooth induction of anesthesia, to maintain adequate cerebral perfusion pressure (CPP) and hemodynamic stability, to avoid venous air embolism (VAE) and to facilitate intraoperative neuromuscular and depth of anesthesia monitoring. It explains the monitors like electrocardiography (ECG), noninvasive blood pressure (NIBP), invasive blood pressure (IBP), pulse oximetry (SpO2), end-tidal carbon dioxide (EtCO2), central venous pressure (CVP) and temperature. The chapter focuses on the drugs like mannitol, anticonvulsant, vasopressors, intraventricular (IV) paracetamol and antiemetics. It also discusses the intraoperative concerns like tense brain, seizure, risk of venous air embolism in semi-sitting position, risk of postural hypotension in semi-sitting position and risk of massive blood loss.
