ABSTRACT

Phalloplasty is the surgical technique of penile reconstruction. Modern phalloplasty surgery reflects the development of surgical techniques in plastic and reconstructive surgery. The tubular shape of the pedicled flap lends itself to phalloplasty. The ideal phalloplasty is one where the phallus, neo-urethra and erection ability or device are constructed in one procedure, with good functional and cosmetic result and with minimal donor site morbidity. Modern phalloplasty techniques consist of three classes of surgery. They are local flaps, pedicled flaps, free flap transfer and microsurgical vessel anastomosis. The simplest local flap is the pubic flap. The pedicled flap came about as a result of treating severe maxillofacial injuries during World War I. The commonest free flap in use today is the radial forearm flap. If a free flap phalloplasty is chosen the feeding vessels must be confirmed to be intact, usually by a vascular Doppler ultrasound examination.
