ABSTRACT

Health providers have long recognized the link between poverty, social deprivation and adverse health outcomes. The connection between poverty and poor health outcomes has been demonstrated across most chronic illnesses, acute illnesses and even accidents and trauma.

This chapter explores further:

How health providers can act on the social determinants of health at various levels (Micro, Meso and Macro).

Using income as an example.

At the Micro level using an anti-oppressive approach, a clinical tool and health team interventions.

At the Meso level through the use of sociodemographic data, health equity assessment tools, and community engagement.

At the Macro level through social policy advocacy.
