ABSTRACT

Advances in medicine, population ageing and the shift to a higher incidence of chronic, non-communicable diseases require that healthcare systems respond to the increasing needs of people living with chronic conditions. Rehabilitation is the health strategy that aims to enable people with chronic conditions and disability to achieve optimal functioning in their environment. This chapter will address a) the unifying model for the discipline of rehabilitation the World Health Organization (WHO) International Classification of Functioning, b) an overview of the rehabilitation assessment, c) a peek into the black box of rehabilitation interventions and d) insights into how rehabilitation outcomes are measured. It concludes with some of the key remaining research questions in the field.
